Font used:

• Myriad Pro pretty standard font, can be replaced with Tahoma or Calibri
• Century Gothic , also a standard font. can be replaced with any sans serif font.

To choose the color scheme, just activate the layer with the color name on the layer box in photoshop, and deactivate the others.
